At Community Companions, we believe independence is built through connection, opportunity, and trust. Our services are designed around each individual’s strengths, preferences, and goals, offering person-centered supports that promote meaningful community participation, skill development, and independence. Through a combination of Community Participation Supports, In-Home and Community Supports, Respite Services, Supported Employment, Transportation and Travel Skills Training, and Person-Centered Planning, we support individuals and families in building sustainable routines, stronger connections, and successful outcomes in everyday life.

Community Companions is a person-centered service provider supporting individuals with intellectual and developmental disabilities across home, work, and community settings. We offer Community Participation Supports, In-Home and Community Supports, Respite Services, Supported Employment, Transportation and Travel Skills Training, and Person-Centered Planning to help individuals build independence, confidence, and meaningful connections. Our services are designed to reflect each individual’s goals, strengths, and daily needs while supporting families and caregivers along the way.

CPS includes:
Volunteering, civic engagement, and leisure activities
Socialization in integrated community environments
Independence skill-building
Travel and safety training
Exploring community resources (libraries, museums, recreation, etc.)
Strengthening informal and natural supports
Staffing: 1:1 or 1:2 support based on the person’s goals and ISP.

Supports include:
Personal hygiene, grooming, and dressing
Meal preparation and household tasks
Budgeting, money management, and shopping
Transportation and community navigation

Ideal for: Individuals who benefit from hands-on support in a familiar environment.

Relief for caregivers, continuity of care for individuals. Temporary support when primary caregivers are unavailable—ensuring safety, engagement, and peace of mind.
Respite includes:
1:1 in-home or community-based supervision
Flexible scheduling (evenings, weekends, overnights)
Recreational and meaningful activities
Behavioral and safety support according to the care plan
Purpose: Give families essential breaks while maintaining consistent support.

Training and support that empower individuals to travel safely and confidently within their community. Services include route planning, public transportation usage, safety instruction, one-on-one travel training, and coordinated transportation to CPS activities, employment, and appointments.
